The type of personal information we collect  

We currently collect and process the following information:

  • Name, address, date of birth, e-mail, contact number.

  • We may also collect medical information which may be necessary for your treatment.  

  • All this information is given to us by you with your consent.    

 

How we get the personal information and why we have it Most of the personal information we process is provided to us directly by you for one of the following reasons:

  • To keep a record of your appointments + treatments.

  • To contact you about future appointments.

  • To contact you for promotions we may have on our services.

  • Send you newsletters.  

 

We also receive personal information indirectly when you visit our website www.dkosteopathy.co.uk. Our website uses cookies. Cookies are used to track information about each users session on the website. This allows us to identify repeat visitors and helps us keep statistical data about our visitors. We use this information to improve our website.

If you do not want this information to be shared, you can change your browser setting to decline cookies. Please note that should you choose to decline cookies, you may be unable to access particular parts of our website.

How we store your personal information  

Your information is securely stored. Only staff engaged in your treatment will have access to patient records. These records are electronically stored and are password protected. The administration team will have access to your contact details only in order to book appointments.

 

We keep personal information for as long as you use our services. When you stop using our services we continue to store this data for eight years. After eight years all data will be deleted. We will keep the records of minors who have had treatment with us until they reach the age of 25.

 

No information will be shared with any third party outside of the organisation unless your consent is given. 

Your data protection rights

Under data protection law, you have rights including:

Your right of access - You have the right to ask us for copies of your personal information.  

Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.  

Your right to erasure - You have the right to ask us to erase your personal information in certain circumstances.  

Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances.  

Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances.

Your right to data portability - You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.

 

You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
